SPRING 2009 REGISTRATION TIMELINE:
- March 11th: Deadline to submit spring 2009 registration paperwork & team entry fee.
- April 1st: Mandatory coaches meeting at the SoccerDome
- April 11th: Spring 2009 outdoor league play begins
- May 30th: Spring 2009 outdoor league play concludes (weather permitting)
Spring 2009 Team Registration Information:
1. MISSOURI YOUTH SOCCER ASSOCIATION (MYSA) Membership Form:
MYSA Registration Form Is required for every player and coach listed on the roster and playing for the team. Please make certain, that parents sign their child’s form at the bottom. Incomplete forms will be returned and not processed. Incomplete forms may delay or prevent children from playing in games.
2. New players are required to submit a copy of the birth certificate along with their MYSA membership form. New players are those who DID NOT play with your team during the fall season.
3. League Fees PAID IN FULL. Refunds for scholarships and house players will be made as soon as possible.
4. DUAL ROSTER DUAL ROSTER REQUEST FORM – If you have a player who wishes to play on 2 different teams at Lake Country Soccer for outdoor leagues, the player MUST have a DUAL ROSTER REQUEST FORM signed by both team’s coaches and submitted with the PRIMARY team’s registration paperwork. Players playing on two teams MUST complete a MYSA Membership form for each team they are playing on. Each team must be competing in different divisions.
5. SPECIAL REQUEST (CONFLICT) FORM No league games will be scheduled on the weekend of April 4-5 due to our Ozarks Coca-Cola / Dr Pepper tournament. Due to the limited number of weekends for the spring season, and to conclude by the weekend prior to the Memorial Holiday Weekend, we cannot accommodate requests for weekends off this spring. There is a possibility that your team could be scheduled to play on a weeknight in addition to the Saturday games, especially in the event of a cancellation of Saturday game(s) due to weather.; except for those teams outside of the 45 mile drive distance radius from Lake Country Soccer.

TEAM CONTACT FORM - This form MUST be completed and submitted with all registration paperwork by the March 11
th deadline. Please be sure the information is correct, as we utilize email as the primary means of communicating with teams.
7. KID SAFE (Risk Management) PROGRAM – Every coach, administrator, manager, trainer and any other adult registering with a team is REQUIRED to register with the Missouri Youth Soccer Association’s KID SAFE Risk Management Program through MYSA’s on line registration system and submit a $15 fee for each team an adult is working with. It is imperative that the adult PRINT THE CONFIRMATION PAGE after completing the online registration process and submit this document along with team registration paperwork. Kid Safe registration is required each registration year.

- v. Print confirmation page
- vi. Submit confirmation page with all registration paperwork
- vii. Each adult listed on the team roster MUST complete this process
- viii. Include $15 for each adult listed on the team roster
General information
8. Player Cards and Rosters
LCS will still be using Player Cards and Roster for league games. Before every league game, Coaches in the major leagues will be required to present the referee with a photo copy of their player signed original roster and cards for all players and coaches. Roster size limits and age restrictions will be enforced. The player cards and rosters will be issued to all coaches at the mandatory coaches meeting on April 1st (Wednesday) at 6:00pm at the Lake Country Soccerdome. A representative must be present for each team participating in the LCS Spring 2009 Outdoor League . The coach/manager is responsible for attaching a picture of each player to his/her player card, along with signature of player on back of card. Cards must be laminated and rosters signed by each player.

Medical Release and Liability Waiver Medical Release Form
The medical release must be kept with the coach at all practices and games. The form must be completed in full, signed by a parent/guardian and notarized. This document is used in the event of injury to the player and the parent/guardian is not present.
10. Roster Changes
Addition and deletion forms to your roster will be dropped off at the LCS Office for processing. Cards and rosters will be available the next business day. There will be no on-site processing of rosters or player passes without an appointment. There will be no on-site processing of rosters or players passes on Saturday's.
11. Scholarships
Scholarships are available to those individuals who receive free or reduced school lunch. Please complete the LCS Scholarship Application Form for those who qualify.

Required Coaching Certification
All coaches at LCS must be certified with a minimum "F" level coaching certificate. Lake Country Soccer will host a MYSA "F" certification course on Monday, February 9th from 6:00pm - 9:00pm. Registrants MUST register online at www.mysa.org and click on "Coaches" and locate the registration tab/hyperlink for this clinic. Coaches with LCS teams are paid for by Lake Country Soccer. Non-LCS coaches MUST pay $35.
13. Web Site Reminder
Just a reminder, e-registration for individuals and team, signup information, league schedules, important dates and downloadable forms can all be found on our web site www.lakecountrysoccer.org. Remember that the on-line registration only notifies LCS of your intention of playing at LCS, all of the paperwork listed above must be submitted to the LCS Office no later than March 11th, 2009 for scheduling purposes. LCS will also update the web site on weather delays and cancellations. Coaches are responsible for communication regarding all reschedule information to their respective players.
14. Practices
Teams that are U-10 and younger can practice on the Lake Country Fields once @ week at no charge.
Teams that are older can practice for a fee. To obtain a scheduled time for your team’s practice you must speak to or email Katie at the Soccerdome, 862-3211 ext 103, or katie@lakecountrysoccer.org.
15. Individual players (house players)
These players sign up with LCS to be placed on a team. Please remember to indicate how many individual players your team can take.
16. Pass Back Program
LCS currently collects soccer cleats which we in turn pass on to those players who are under financial constraints. Please contact the LCS Office for assistance with this program.
