MYSA - LCS Membership Registration Form This form is an interactive, typeable form.  You may type information directly into the form on your desktop then print.  No information typed into the form is saved.  this form MUST be notarized before submitting to Lake Country Soccer.   This form is to be used for all new players, coaches and managers for outdoor youth soccer. Coach Registration  New coaches must register and submit a background check verification form before being assigned to teams or listed on any team roster.  Registration is managed by the Missouri Youth Soccer Association and Gotsoccer.com. When completing the MYSA.org "F" clinic registration (www.mysa.org), if you are coaching in the Lake Country Soccer league, Lake Country Soccer pays the course registration fee.  When you reach Step 4 (payment) in the clinic/class registration system, select the "pay by check" option.

Each NEW player to Lake Country is required to submit a completed MYSA/LCS  registration/membership form.  Liability consent/ medical release forms MUST be notarized and kept with the team coach or team manager and available at all practices and games.

Fall 2010 Team Registration:

1. MISSOURI YOUTH SOCCER ASSOCIATION (MYSA) Membership Form:
 MYSA - LCS Membership Registration Form Is required for NEW players and coaches listed on the roster and playing for the team. Please make certain, that parents sign their child’s form at the bottom. Incomplete forms will be returned and not processed. Incomplete forms may delay or prevent children from playing in games. RETURNING players and coaches DO NOT have to submit paperwork for the 2010/2011 seasonal year

2. New players are required to submit a copy of the birth certificate along with their MYSA membership form. New players are those who were not registered through LCS during the 2009-2010 registration year.

3. League Fees PAID IN FULL. 

4. DUAL ROSTER   DUAL ROSTER REQUEST FORM – If you have a player who wishes to play on 2 different teams at Lake Country Soccer for outdoor leagues, the player MUST have a DUAL ROSTER REQUEST FORM signed by both team’s coaches and submitted with the PRIMARY team’s registration paperwork. Players playing on two teams MUST complete a MYSA Membership form for each team they are playing on. Each team must be competing in different divisions.

5. SPECIAL REQUEST (CONFLICT) FORM Each team is allowed to request 2 weekends off. If you request dates off, your team will be scheduled to play 1 to 2 double header Saturdays or will be scheduled on one or more weeknights. League games begin on Saturday, August 28th and tentatively scheduled to conclude on Saturday, October 30. NO GAMES SCHEDULED FOR SAT. OCT. 16 and SUN. OCT. 17 due to the Lake Country Soccer / Incredible Pizza Fall Classic Tournament

Coaches: It is your responsibility to notify Lake Country Soccer, of the dates your team is NOT available to play by the July 30^th deadline date. The requested days off MUST be noted on the online application. There is a possibility that your team could be scheduled to play on a weeknight in addition to the Saturday games, unless it is noted as to which weeknights your team is NOT available to play.

*******NEW POLICY******If your team requests a schedule change that was not included in the online application, after the schedules are published and the schedule change is approved by  LCS management, your team will be assessed a $25 rescheduling fee per game. 

6. TEAM CONTACT FORM - This form MUST be completed and submitted with all registration paperwork by the July 30^th deadline. Please be sure the information is correct including your email address, as we utilize email as the primary means of communication.

****** 2010/2011 BACKGROUND CHECKS ON COACHES & TEAM MANAGERS ******

Coaches will need to process their background check through the Missouri Youth Soccer Association's online registration program, Got Soccer.  If you do not already have a coach, manager, or referee account with Got Soccer, you will need to have an account set up for you through whichever club you register your players with. OR set up an account for yourself.  If you coach or manage in more than one club, you will need multiple accounts set up with each club.  Lake Country Soccer is a singular club, therefore, you would only need one account if your name is to appear on multiple team rosters to coach for any Lake Country Soccer team. Once your account is created, here are the step-by-step instructions for completing the BACKGROUND CHECK for 2010/2011: - Login to your account - In the gray bar along the top, you will see a tab entitled "Background Checks". Click it. - A screen will come up that has a button entitled "Submit New Report:.  Click it. - You will then be prompted to enter in your personal information and answer questions concerning your legal history. - Once you've completed filling-in your information and answering your questions, you will submit the report. - The report will be sent to us for verification and the check usually is complete within 1-2 days. - After you are approved, you will be able to be added to your team or event. For Referees: You may now go online and create an account in the Gotsoccer.com system. In order to process your background check, you must be assigned to a referee assignor recognized by MYSA.  Otherwise, you will not have the ability to process a background check.  Please contact the referee assignor you are associated with to have your account set up if you do not have one already.  You will not be able to process the background check unless you are attached to the state account or a referee assignor who is attached to the state.

 

General information

8. Player Cards and Rosters

LCS will utilize league issued player Cards and league roster for league games. Before every league game, Coaches in the major leagues will be required to present the referee with a photo copy of their player signed original roster and laminated cards for all players and coaches. Roster size limits and age restrictions will be enforced. There is a mandatory coaches meeting on Thursday August 19, 6:00pm at  Incredible Pizza. A representative must be present for each team participating in the LCS Fall 2010 Outdoor League .

9. Medical Consent and Liability Release Waiver  Medical Release Form

The medical consent / liability release waiver must be kept with the coach at all practices and games. The form must be completed in full, signed by a parent/guardian and notarized. This document is used in the event of injury to the player and the parent/guardian is not present.  The release is valid for as long as all information on the form is correct.

10. Roster Changes

Addition and deletion forms to your roster will be dropped off at the LCS Office for processing with a completed Roster Change Form which can be found above. The revised team list must also accompany the Roster Change Form. Cards and rosters will be available the next business day.   There will be no on-site processing of rosters or player passes without an appointment. Any player being dropped from a team must turn in player card at time of change.

11. Scholarships

Scholarships are available to those individuals who receive free or reduced school lunch. To apply for a scholarship, please submit a completed scholarship application and supporting documentation with the child's MYSA Membership Form.  Application DOES NOT guarantee a scholarship award. 

12. Required Coaching Certification

All coaches at LCS must be certified with a minimum "F" level coaching certificate. An "F" License clinic will be held at the LCS Soccerdome on Tuesday August 17, 2010 from 6:00pm -9:00 pm and is paid for by Lake Country for those who are coaching teams in the LCS leagues. We MUST have a minimum of 16 attendees.  Coaches must register for the course through Missouri Youth Soccer’s web site (MYSA.ORG).  When completing the MYSA.org "F" clinic registration, if you are coaching in the Lake Country Soccer league, Lake Country Soccer pays the $35 course registration fee.  When you reach Step 4 (payment) in the class registration system, select the "pay by check" option. 

13. Web Site Reminder Just a reminder, e-registration for individuals and team, signup information, league schedules, important dates and downloadable forms can all be found on our web site www.lakecountrysoccer.org. Remember that the on-line registration only notifies LCS of your intention of playing at LCS, all of the paperwork listed above must be submitted to the LCS Office no later than July 30th for scheduling purposes. LCS will also update the web site on weather delays and cancellations. Coaches are responsible for communication regarding all reschedule information to their respective players.

14. Practices

Teams that are U-10 and younger can practice on the Lake Country Fields once a week at no charge.

Teams that are older can practice for a fee. To obtain a scheduled time for your team’s practice you must speak to or email Katie at the Soccerdome, 862-3211 ext 103, or katie@lakecountrysoccer.org.

15. Individual players (house players)

These players sign up with LCS to be placed on a team. Please remember to indicate how many individual players your team can take if your team is looking for additional players to fill a roster.

16. Pass Back Program

LCS currently collects soccer cleats which we in turn pass on to those players who are under financial constraints. Please contact the LCS Office for assistance with this program.
