2009 spring referee availability forms MUST be submitted to Tonya Wade-Smith to be eligible to officiate LCS youth league games.  Also, every referee is REQUIRED to have on file at Lake Country Soccer, a completed 2009 IRS Form 1099-Misc.

Referees under the age of 16 MUST have a completed PARENTAL CONSENT form on file with Tonya Wade-Smith.
